[4]-Your object should like this

[5]-Create a box with the following size
(Length: 34.0) (Width: 19.0) (Height: 9.0)

[6]-Now convert into the Editable poly and select the Edge mode
With the Ignore Back facing

[7]-Select the Edges like this in the top view port
Apply the Extrude Edges and use the following setting

[8]-Select the polygon mode and select the polygons in the top view port
Apply the Extrude polygons and use the following setting

[9]-Your polygon should like this in the front view port

[10]-Now repeat the step [8]

[11]-Your polygon should like this in the front view port

[12]-Create a box and Boolean like this

[13]-Now your object should like this in the perspective view port

[14]-Create a chamfer cylinder and Boolean the box

[15]-Now your object should like this in the perspective view port

[16]-Select the Second chamfer cylinder and apply the Taper Modifier
Use the following setting your object should like this in the perspective view port

[17]-Open the material window and click on the diffuse map button
Now select the bitmap option and pick the jpg like this

[18]-Now render the Perspective view port and you will see the final image is ready
Save the file in your folder
