Now this time apply only extrude polygon with (Extrusion amount: 1.0) and Local Normal

[8]-Create a rectangle with following size
Length: 90
Width: 48
Corner radius: 5.0
Set like this in the top view port

[9]-With still select the rectangle and open the rendering palette
Then use the following setting

[10]-After use the following setting your object should like this in the in the top view

[11]- Create a chamfer box with following size
(Lenght: 6.0)
(Width: 32.0)
(Hieght: 58.0)
(Fillet: 11)
Create a chamfer box like this in the top view

[12]-Select the box and convert into the Editable poly
Select the vertex mode with Ignore Back facing and select the vertexes like this in the top view port
And left view port now press the Delete button

[13]-Apply the Shell modifier with the (Outer Amount: 1.0)

[14]-Now your image should like this in the perspective view port

[15]-create Circle with following size
Radius: 2.0
Open the Rendering palette
Select the Enable in Rendering and Enable in View port
Set the (Thickness: 0.44)

[16]-Now create a five more circle and set like this

[17]-Create a Text (123456789) like this and apply the Extrude Modifier
Set the Text like this in the top view port

[18]-Open the Material window and make three materials like this

[19]-Now click on the diffuse map button and select the bitmap option
Now pick the Jpg like this and assign to the screen

[20]-After assign the material press (f9) and you will see the final image complete
Save the file in your folder
