[6]-Again create rectangle and set like this

[7]-Select the first rectangle and convert into the Editable poly
Select the Edges mode

[8]-Select the edges like this and apply the Chamfer Edges and use the following setting

[9]-Now click on the line button and create a one more copy
With the help of Mirror palette now set like this

[10]-Similarly create one more copy and set like this

[11]-Create a two line and one circle
Sets like this

[12]-Open the material window and a material like this

[13]-This is final result
