Select the polygon like this
Apply the bevel for two times

[10]-Open the material window and click on the diffuse map button
Now select the Gradient and use the following setting
Create one more material also

[8]-Create a WRectangle and set like this

[9]-Create a three cylinder with following size

[10]-your object should like this in the perspective view port

[11]-Select the rectangle and create a one more copy
Set like this

[12]-Convert into the Editable poly and select the Polygon mode
Select the all polygons and click on the Slice Plane Button
Set the plane like this

[13]-Now click on the slice button like this
You will see the new segment are create
Same technique for other segment

[14]-Now after the segments apply the Extrude Edges and use the following setting
Your object should like this

[15]-Create a two Tube like this

[16]-Select the both tube and convert into the Editable poly
Select the polygon with the Ignore Back facing

[17]-Select the polygons like this and apply the Extrude Polygons
And use the following setting

[18]-Your object set like this

[19]-Open the material window and make a material like this

[20]-Now render the perspective view port and you will see the final image is ready

Now convert into the Editable poly and select the vertex mode

[9]-Click on the line button and create a shape like this in the front view

[10]-Now apply the Extrude Modifier and set the (Amount: 0.3)
And create three copies of the shape and set like this in the front view

[11]-Click on the Chamfer box button
Create a Chamfer box with the following settings

[12]-Select the box and convert into the Editable poly
Select the vertex mode with Ignore Back facing and select the vertexes like this in the top view port
And left view port now press the Delete button

[13]-Apply the Shell modifier with the (Outer Amount: 1.0)

[14]-Now your image should like this in the perspective view port

[15]-create Circle with following size
Radius: 2.0
Open the Rendering palette
Select the Enable in Rendering and Enable in View port
Set the (Thickness: 0.44)

[16]-Now create a five more circle and set like this

[17]-Create a Text (123456789) like this and apply the Extrude Modifier
Set the Text like this in the top view port

[18]-Open the Material window and make three materials like this

[19]-Now click on the diffuse map button and select the bitmap option
Now pick the Jpg like this and assign to the screen

[20]-After assign the material press (f9) and you will see the final image complete
